Application
The Harold Amos Medical Faculty Development Program supports
basic/biomedical, clinical, and health services/epidemiology research.
The purpose of the AMFDP award is to facilitate the transition of the
newly-trained clinician who wishes to develop into an independent investigator.
The information requested in the application process is essential for
screening as well as for interviewing applicants.
The application process will be partially online, through the
Robert Wood Johnson Foundation's Grantmaking Online system, found after January 8th at http://grantmaking.rwjf.org/index.jsp . Please note that in
this system, your application is termed a proposal, and your research
proposal is called a proposal narrative.
Reapplicants please note: It will be necessary for this submission
to choose an ID and password that are different from those used for the
2003 through 2007 proposals in the Grantmaking Online system.
A completed application consists of two parts: the completion
of the online proposal process, and submission of other (paper) supporting
documents. Click here for a checklist of documents you will need.
The National Advisory Committee will consider applications only
when they are complete. Therefore, you should request the following as
soon as you decide to apply:
References: Download the Word version of the Confidential Reference Report and forward it to each of three people, or point them to the Confidential Reference Report link here. To ensure
strict confidentiality, the completed forms should be mailed directly to the National Program Office postmarked no later than March 20, 2008.
They should not be returned to the applicant. One of the referees must
be someone familiar with your research interests and/or experience. Your
proposed Mentor(s) should not be used as a reference.
Transcripts: Please request that your undergraduate college(s),
medical school(s), and any institution at which you undertook health-related
coursework (toward an MPH or PhD, for example) forward your transcripts
directly to the AMFDP National Program Office postmarked no later than
March 20, 2008.
Mentors: You must reach an understanding with a prospective
Mentor who has substantial research experience and who is
willing to provide guidance for your research training. Download the
Letter to the Proposed Mentor and give it to
him or her. This letter should serve as a guide to the kind of information
we need from your Mentor(s). Successful competition for this award
is strongly influenced by the quality and strength of your proposed
Mentor. Please be sure that you have substantive contact with him or
her as you develop and complete your application. If you have more
than one mentor, they may collaborate on one letter, or may provide
separate letters; however both must be uploaded to the Grantmaking
Online system together as a single document.
Next, visit http://grantmaking.rwjf.org/index.jsp after January 8th and register with the site so that you may begin to submit your proposal.
At the site, you will be asked five preliminary questions. One of
them asks if you have read the Quickstart
information and FAQs. You
will also be asked to provide contact information for yourself and
your mentor, as well as information about your institution and background.
We also require a short, readable executive summary of your proposal,
in no more than 1500 characters. Sample
executive summaries are provided
to guide you in developing your summary.
You will be uploading documents that you create to the Grantmaking
Online system, using the templates provided. Each of these
has a 15MB size limit. These documents are as follows:

Mentor's CV
Co-Mentor's CV (required only if you have two mentors)
Mentor's (Mentors') Statement
Supplemental Documents (optional)
Proposal Narrative: There are four substantive questions to answer
and upload as part of the online process. Click here for a preview
of these questions.
You will be asked to upload as a separate document the references/citations
used in developing your scientific research proposal. These citations
will not count toward the six-page limit for the description of your
research project.
Next, complete a template and upload your CV to the system.
You may format your CV in any way you want, but please include at a
minimum the following information: undergraduate, medical, and other
institutions, major(s), degrees, dates, and honors; internships and
residencies; fellowships; research and work experience; academic appointments;
and publications. Please use 1-inch margins and a 12-point font.
You will also be asked to complete a template and upload your
Mentor's CV. The NIH biosketch format is preferred; however,
the CV may be formatted in any way you want. In the case that
you have more than one Mentor (and most applicants do not),
use the Co-Mentor CV template to upload a second CV/biosketch.
Mentor's (or Mentors') Statement: You must upload a statement
from your Mentor or Mentors as noted above. In essence, this statement
contains the answers to the questions posed in the Letter to the Proposed
Mentor that you downloaded.
Supplemental Documents: You may complete this template if you
will be submitting supplemental documents. Most applicants will not
have supplemental documents. Examples of these documents could be the
CV of someone who is providing critical expertise or support to the
project but who is not a Mentor or Co-Mentor, or a letter of support
from a chair or dean. If you have any question about the appropriateness
of including a specific document, please contact the National Program
Office. There are no restrictions for length, spacing, font size, or
margins for the Supplemental Documents; however, the 15MB limit applies.
You may copy and paste from an existing document into this template.

Twelve Sets of
Paper Copies: Twelve sets of paper copies of all the proposal documents
you have uploaded to this Grantmaking Online system are required
in addition to the online submission. You should print your Proposal
Summary (see link on lower left side of your screen). Note that you
will also need to separately print out your Proposal
Narrative (Research
Training, Interest/Intent, Commitment to the Goals of the AMFDP,
and Project), References/Citations, Applicant
CV , Mentor CV , Co-Mentor
CV (if applicable), Mentor's
Statement, and Supplemental
Documents.
Your proposal packet will consist of twelve collated sets
of the following, sent to the National Program Office postmarked
no later than March 20, 2008:
- Proposal Summary (which includes the Executive
Summary)
- Proposal Narrative (Research Training, Interest/Intent,
Commitment to the Goals of the AMFDP, Project)
- References/Citations
- Applicant CV
- Mentor's CV (NIH-style biosketch preferred)
- Co-Mentor's CV/biosketch, if applicable.
- Mentor's statement
- Supplemental
documents (if applicable)
Keep in mind that we require
that you provide twelve copies of everything that
will be reviewed, with the exception of transcripts
and references, which should be received separately.
